CCS Service Facilitator
Looking for a flexible position in the mental healthcare space working with a team of professionals in a modern and collaborative environment? Join our Greyley Wellness service facilitation team working within the Dane County Comprehensive Community Services program.

A Service Facilitator/Case Manager is responsible for coordinating recovery-based services for clients who face significant mental health and/or addiction challenges. Service Facilitators develop assessments, recovery plans, make referrals for services, facilitate team meetings, and much more. The ideal applicant can thrive in a flexible, supportive environment and is a motivated self-starter.
